Privacy Policy

AMS Wellness & IV ("we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect through our website at www.amswellnessandiv.com (the "Site") and in connection with our clinic services, how we use it, and the choices you have. This policy covers the Site and our general business information practices. How we handle your medical records as a patient is described separately in our Notice of Privacy Practices.

Who we are

AMS Wellness & IV is a physician-supervised, in-person wellness clinic and medical spa with two Florida locations and a mobile IV service that comes to homes, hotels, offices, and events in the Sarasota and Englewood area. Our Medical Director and Supervising Physician is Quratulain Hayat, DO, Family Medicine, Florida License OS15796. We are a brick-and-mortar practice. We do not offer telemedicine, we do not prescribe online, we do not sell medication online, and we do not ship medication.

Adults only

Our services and this Site are intended for adults 18 and older. We do not knowingly collect personal information from anyone under 18, and the Site is not directed to children under 13. If you believe a child has given us personal information, contact us at contact@amswellnessandiv.com and we will delete it.

Information we collect

Information you give us. When you use our contact form, book an appointment, complete intake or consent forms, or email us, you may provide your name, email, phone number, and details about the services you are interested in.

Booking and intake information. We use Zenoti, a third-party scheduling and clinic-management platform, for online booking and for health intake and consent forms. When you book or complete forms online, that information is collected and stored by Zenoti on our behalf. You can also complete forms in the clinic on paper or a tablet.

Information collected automatically. Like most websites, our Site automatically collects certain technical information such as your IP address, browser type, device information, pages visited, and the dates and times of your visits. We use Google Analytics to understand Site traffic.

Tools on our Site

AMS Guide chat helper. Our on-page chat helper, AMS Guide, is rule-based and runs entirely in your browser. It does not store your conversations and does not send them anywhere.

Educational quizzes. Our on-page quizzes (drip finder, hormone quiz, peptide quiz) run entirely in your browser and store nothing on a server. If you use a "copy my summary" button, the text is copied only to your own device's clipboard.

Google Fonts and Google Maps. We use Google Fonts to display text and link to Google Maps so you can find our locations. When these load, Google may receive your IP address.

Educational site. We link to a separate educational site, amstreatmentguide.com, which is provided for general information.

Cookies and analytics

Our Site uses cookies and similar technologies, including Google Analytics, to measure traffic and improve the Site. You can set your browser to refuse cookies or alert you when cookies are being sent. You can opt out of Google Analytics on any website by installing the Google Analytics Opt-out Browser Add-on, available from Google at tools.google.com/dlpage/gaoptout.

How we use your information

We use the information we collect to respond to your inquiries; schedule and manage appointments; provide and improve our services; operate and secure the Site; comply with legal obligations; and, with your consent, send you marketing communications.

Marketing emails and text messages

We send marketing emails and text messages (SMS) only to people who opt in.

Email. You can unsubscribe at any time using the link in any marketing email. Consistent with the federal CAN-SPAM Act, we honor opt-out requests within 10 business days, and every marketing email includes our valid physical mailing address.

Text messages (SMS). By providing your mobile number and opting in, you agree to receive marketing text messages from AMS Wellness & IV at the number provided. Consent is not a condition of purchase. Message frequency varies. Msg & data rates may apply. Reply STOP to unsubscribe at any time, or reply HELP for help. We handle SMS consent consistent with the federal Telephone Consumer Protection Act (TCPA), which requires your prior express written consent before we send marketing texts.

How we share information

We do not sell your personal information. We share information only with service providers who help us operate (for example, Zenoti for scheduling and intake, our website host Squarespace, and Google for analytics and fonts); with other parties when you direct us to; and with authorities when required by law. Our service providers are permitted to use your information only to provide services to us.

Data security and breach notice

We take reasonable measures to protect the information we hold. If a data breach affecting Florida residents' personal information occurs, we will notify affected individuals as required by the Florida Information Protection Act (Fla. Stat. 501.171). Under that law, notice must be given as expeditiously as practicable and no later than 30 days after we determine a breach occurred, and we will notify the Florida Attorney General if the breach affects 500 or more Florida residents. Because some health-related information collected through the Site may fall outside HIPAA, we may also have obligations under the FTC's Health Breach Notification Rule.

Your choices and state privacy notes

California (CCPA/CPRA). We are a Florida-only clinic and do not meet the thresholds that make the California Consumer Privacy Act apply. If this changes, we will update this policy.

Florida Digital Bill of Rights. Florida's comprehensive privacy law (Fla. Stat. 501.701–501.722) applies only to very large companies that meet specific revenue and business-model criteria. It does not apply to us.

Do Not Track. Some browsers offer a "Do Not Track" signal. Because there is no common industry standard for it, our Site does not currently respond to Do Not Track signals.

Retention

We keep information for as long as needed for the purposes described here and as required by law. Patient medical records are retained under Florida law as described in our Notice of Privacy Practices.

Changes to this policy

We may update this Privacy Policy from time to time. The "Last updated" date below shows when it last changed.
